In case of Emergency for Brian!  

 

1.) call 911

2.) notify paramedics:
- Brian’s blood type: A+
- Organ donor
- Date of Birth: Oct 2, 1972
- Medical Insurance: Blue Cross/Blue Shield Federal Employee Program; Member ID R57864173; Enrollment Code 104

3.) If death is likely, please contact a Roman Catholic Priest to conduct Last Rites (St. Lawrence Parish, Alexandria, VA: 703-971-4378).  If they are too far from the scene, they can locate and send the closest Priest.

4.) call Brian’s wife, Shannon May, at 703-861-0607. If Shannon isn’t available, contact James Wilt at 301-751-9691, or Nancy Wilt at 301-466-7034.

5.) If death occurs, please contact the place where Brian has donated his body to science:
